A number of presentations of PROactive data were given at the American Diabetes Association (ADA) 66th Annual Scientific Sessions held in Washington DC, USA on 9–13 June 2006. They showed that, compared with placebo, long-term pioglitazone treatment given in addition to existing cardiovascular and glucose-lowering medication in patients with type 2 diabetes and macrovascular disease resulted in:
- Effects of Pioglitazone on Major Adverse Cardiovascular Events (MACE) and Myocardial Infarction: Results from PROactive

Compared with placebo, pioglitazone therapy resulted in clinically and statistically significant risk reductions in the time to major adverse cardiovascular events defined as cardiovascular death, non-fatal MI, or non-fatal stroke (18%) and time to fatal or non-fatal MI (23%).

- Long-term Pioglitazone Treatment Improves Markers of Liver Function: Results from PROactive

In the pioglitazone group, there was an overall shift toward normalisation from baseline in alanine aminotransferase (ALT), a marker of the amount of liver fat related to liver insulin resistance. A decrease of 5.9% in the occurrence of elevated ALT was noted in the pioglitazone group compared with a small increase in the placebo group (11.9%). This suggests that treatment with pioglitazone is associated with a lowering of liver fat content.

- Reduced Insulin Requirements and Improved Glycemic Control with Pioglitazone in Insulin-treated Patients with Type 2 Diabetes: Results from PROactive

In the one-third of patients in PROactive who were treated with insulin at baseline, pioglitazone provided sustained glycaemic control. Insulin was discontinued in 9% of patients in the pioglitazone group compared with 2% in the placebo group, and there was a lower mean insulin dose in the pioglitazone group at final visit (42 U/day versus 55 U/day). - Pioglitazone in Triple Oral Therapy: Long-term Glycemic Results from PROactive

Increasing treatment from a dual therapy regimen (metformin plus sulfonylurea) to a triple therapy regimen by adding pioglitazone resulted in sustained glycaemic control and a reduced need for insulin. In the piogitazone group, 16% had either metformin or sulfonylurea dropped from their regimen (versus 8% in the placebo group), while 16% had insulin added (versus 31% in the placebo group). - Pioglitazone Delays the Need for Permanent Insulin Use: Results from PROactive

Double the number of placebo-treated patients (n=362) progressed to permanent insulin use versus pioglitazone-treated patients (n=183).
